This assessment may be suitable if your child needs a clearer picture of their functional skills, adaptive functioning and support needs across daily activities at home, school or in the community. The assessment considers your child’s everyday functioning and the practical skills they use across daily routines.

- Intake or background information session
- Direct assessment or testing session(s)

Not included
- This assessment is not covered by Medicare.
- Home or school visits are not included unless arranged separately; additional charges may apply.
- Additional work outside the package scope may incur extra fees.
How it works
Assessment Process
The assessment process is broken into clear sessions so the clinician can gather background information, complete the required assessment tasks, prepare the report and discuss the results with you.
Session 1: Parent / Guardian Intake Session
1 hour session. Information gathering session.
Includes a review of your current goals in your NDIS plan (or development of goals if you are applying for NDIS), supports currently receiving, and identifying ongoing goals. This can be conducted in clinic or via Telehealth.
Session 2: Child Assessment Session
1 hour session with the child and parent. This session must be completed in clinic.
Session 4: Parent / Guardian Feedback Session
1 hour session. This can be completed in clinic or via Telehealth.
Report timeframe: 4 to 6 weeks from assessment date, depending on complexity.
Comprehensive Report
The clinician completes scoring, interpretation and report preparation after the assessment sessions.
What to expect
Report and Fees
A comprehensive report is provided including scoring and interpretation of the tools used, addressing NDIS Goals, current supports, NDIS Criteria and summarising all assessment results. Total cost: From $2070.
A $550 non-refundable deposit or signed NDIS Service Agreement is required at the time of booking. This assessment is not covered by Medicare.
